New Patient Registration
Registration forms are available to complete here. Both forms must be completed for each patient. Completed forms should be e-mailed to us as firstname.lastname@example.org
From 1 June 2022 patients are free to request to be registered with any GP Practice which covers the area they live in. If the practice is full at the time we receive your registration request, we will contact you and give information on how to contact Aberdeen City Primary Care Team who can place you on a waiting list for the practice.

You need to complete both of the above online registration forms for each person registering.
Please complete these as fully as possible.
The online new patient health questionnaire allows us to provide medical care in the interim period while your medical records are transferred from your old practice to us.
Please provide us with records of immunisations, especially those for children, as your child may be immunised unnecessarily without this information. Please provide these with you even if they are in a language other than English. These can be posted to us at the practice or you can contact reception on 01224 492828 to arrange to email them to us.
Preference of Practitioner
Patients are registered with the practice, not with an individual GP and your medical card will be issued in the name of the practice. However, you can, at any time, express a preference for a practitioner. Please note however that not all the doctors in the practice provide all services and specific doctors may not be immediately available.
