New Patient Registration
A new registration will be processed within 3-4 weeks of being received - we will text you to let you know when you have been registered
We are accepting registrations from patients who are not registered with a GP Practice where they currently live (ie those who have moved out of their previous GP Practice boundary or have moved to the UK and have not yet registered with a doctor). In line with current arrangements across Aberdeen City - we do not accept new patients if you are already registered with a practice that covers the area where you live.
To register - you must complete BOTH our Registration Form and Patient Health Questionnaire and get them to us at the same time. It is important that we have the correct information when registering patients, so we would recommend that you complete and send these forms to us electronically. If you are not able to do this, please make sure the form is completed in block capitals and can be easily read.

Should our reach the maximum number of patients that we can safely deal with, we will let you know that we are not accepting registrations at that time and ask that you contact an alternative GP Practice or check with Practitioner Services, NHS Grampian (01224 358400) for information on practices that are open and accepting new patients.
You must complete both the Registration Form and a Patient Health Questionnaire before sending these to us (unfortunately, we cannot process your registration if both are not received) -
